Scientific work suggests that the oldest exposed lava flows on Barren Island were erupted around 1.6 million years ago, placing the volcano well within the Quaternary history of the Andaman Sea. After roughly one and a half centuries of relative quiet, the volcano reawakened in 1991 in an eruption that lasted about six months and caused significant ecological damage. A Geological Survey of India expedition in April 1993 found that the eruption had reduced both the diversity and abundance of the island's birds, with only six of sixteen previously recorded species detected; the pied imperial pigeon was the most numerous survivor. The same visit recorded a single rat species and around fifty insect species across eight orders, and confirmed that the volcano was still degassing. Further eruptions were recorded in 1994 to 1995 and from 2005 to 2007, the latter activity being linked to the catastrophic 2004 Indian Ocean earthquake.

Activity resumed in 2017, when scientists from the National Institute of Oceanography witnessed short episodic eruptions with ash plumes during the day and red lava fountains after dusk.
